In making a demand for the refund of amount pursuant to Sub-section (3) of Section 113 of the Act, application shall be submitted to the department in such format as prescribed by the Department, and accompanied by the  document supporting that the amount of which refund is demanded is
over-plus and also by such other documents as prescribed by the Department.
